Phoenix Family Practice is looking for a Practice Nurse to join our patient focused team.
We are seeking a skilled and enthusiastic nurse with chronic disease experience to join our nursing team.

Responsibilities
To provide high quality, evidence-based nursing care to the practice population, supporting health promotion, disease prevention, and the management of acute and long-term conditions in accordance with local and national guidelines.
The successful candidate will ideally have experience in:
- Wound care management
- Cervical screening, contraception, women’s health
- Management of long-term conditions e.g asthma/COPD, diabetes, CHD
- Immunisations and vaccinations (including childhood and travel)
- Health promotion and patient education
- Participation in infection prevention control and clinical governance activities
- Accurate clinical record keeping using clinical system (e.g EMIS / ACCURX)
Requirements
- Active NMC registration
Beneficial
- Independent prescribing
- Specialist Level 7 scope of practice
