We’re looking for a motivated and patient focused PCN Clinical Pharmacist to join our friendly and supportive PCN team. This role is ideal for a pharmacist who thrives in a multi disciplinary environment and is passionate about medicines optimisation, high quality patient care, and collaborative working across practices.

You’ll be fully supported by our Lead PCN Pharmacy Technician, Lead Clinical Pharmacist, the wider pharmacy team, and the practice GPs.
Responsibilities
As a Clinical Pharmacist within the PCN, you will:
- Run long term condition clinics (e.g., asthma, COPD) and carry out structured medication reviews
- Support patients with medicine optimisation, adherence and understanding of their treatment
- Handle medicines queries from patients, GPs, and practice teams
- Support repeat prescribing, medicines reconciliation, safer prescribing systems, and discharge-related medication reviews
- Manage common/self limiting conditions within scope of competence
- Contribute to medicines safety, quality improvement, audits, and implementation of local/national prescribing guidance
- Support CQC compliance regarding medicines and contribute to public health initiatives
- Work closely with the multidisciplinary team, attending MDTs where appropriate
Requirements
Essential
- MPharm (or equivalent) and GPhC registration
- Hands on post qualification experience
- Strong clinical knowledge and medicines optimisation understanding
- Excellent communication skills and the ability to work collaboratively across multiple sites
- Membership of a pharmaceutical body
- Ability to prioritise effectively and recognise when to escalate
Desirable
- Experience working in a GP practice / primary care setting
- Familiarity with SystemOne or EMIS (not essential – training can be provided)
- Experience supporting structured medication reviews and LTC clinics
- Working towards or holding an Independent Prescribing qualification
