Job summary
We are seeking GPs to provide senior medical support to the Intermediate Care Ward (ICW) at Conquest Hospital, with immediate cover required. This is a GP-led ward supporting patients who are medically fit for discharge but awaiting community care packages.
We are looking to recruit GPs to work between 1–5 days per week, as part of our existing GP pool. We are flexible and happy to work around individual availability and existing commitments.
This post is offered on a contract of 6 months to 1 year.
You will work closely with nursing staff and the wider multidisciplinary team to ensure patients remain clinically stable and continue progressing towards rehabilitation and reablement goals. The role involves providing senior clinical oversight, supporting decision-making and prescribing as required, and contributing to safe, timely discharge planning. The role can sit comfortably alongside other GP commitments.
The ICW plays a vital role in reducing acute hospital pressures by offering safe, short-term transitional care for older adults with frailty, mobility needs, and social care dependencies.
Responsibilities
- Provide senior medical leadership on the ward
- Offer clinical oversight, prescribing, and decision-making support for concerns raised by the ward team
- Prescribing and documentation are completed using ESHT systems, including EPMA, NerveCentre, and E-Searcher
- Ensure patients are clinically stable and progressing appropriately towards discharge
- Support a cohesive, multidisciplinary recovery plan tailored to each patient
- The role focuses on oversight rather than acute intervention
Working pattern
- On-site ward round: 9:00am – 12:00pm (3 hours)
- Non-urgent on-call phone support until 4:00pm for infrequent follow-up queries, enabling the GP to continue with other clinical commitments
- Ward operates Monday–Friday (excluding bank holidays)
- Any time between 1-5 days as part of our existing GP pool, this can be flexible and discussed with us
