Job summary
This is a great opportunity for a registered Paramedic/Nurse practitioner or a trainee advanced care practitioner who is currently undertaking Advanced Care Practitioner training. You will work in a friendly, multi-disciplinary team, working alongside GPs assessing and triaging patients in the minor illness clinics and conducting home visits. This will include same day triage and as appropriate provide a definite treatment or make necessary referrals to other members of the primary care team.
Responsibilities
You will also:
- Act as a key contact for Care Homes and patients on the frailty register, offering proactive care.
- Advise patients on general healthcare, including self-management and signposting,
- Undertake the collection of pathological specimens,
- Support the team with clinical emergencies,
- Liaise with external services/agencies to ensure patients are supported appropriately (vulnerable patients etc)
