The Royal College of General Practitioners has published the Fairer Practice Toolkit, a practical, evidence-informed resource to help general practices reduce health inequalities.
The toolkit offers over 100 actions across 18 themes, organised into four progressive tiers. Practices identify their own “Focus Patient” group – those most at risk of poor access and outcomes locally – and choose actions relevant to their context. The toolkit draws on research including the Core20PLUS5 framework and is free to access. The Fairer Practice community has created an online tracker to help practices log their progress
