Surrey and Sussex LMCs wish to bring to your attention the release of national draft proposal documentation and consultation opportunities on the proposed Multi-Neighbourhood Provider (MNP) and Single Neighbourhood Provider (SNP) model. We had been expecting that actual contract details would have been released by now, but there is to be a consultation first.
We have attached the documentation that was released, which includes an FAQ for your ease.
The key highlights of these documents outline:
PCNs/SNPs
- Strengthen PCN & SNP contracts in the interim and protecting income in ARRS
- Short term ability to transition from PCNs to new SNP models
- Eligibility criteria of access to registered list
- SNPs must be a legal entity (although could be held by a lead or host practice) therefore each practice does not have to form separate legal entities.
MNP
- Two types of models:
- Coordination Model
- Lead Provider Model
- MNPs will be commissioned via standard NHS contract forms
- MNPs of multiple organisations will need a host or lead to hold the contract
- Ability to sub-contract from MNP for single neighbourhood level services
- Procurement abilities/restrictions will depend on the contract requirements.
Practices are encouraged to review the full documentation and provide feedback via the national link (below) by 10th September 2026.

The LMC has reached out to local ICB leadership teams and will also, in due course, meet with the Primary Care provider collaboratives to discuss future models being explored to align with the national direction of travel.
