Job summary
Adur Health Partnership (AHP) has an exciting opportunity for a clinical pharmacist to lead their pharmacy team.The role is predominantly clinical, providing face to face and phone medication reviews. It is advantageous, but not essential, for the candidate to be an independent prescriber and have completed the CPPE pathway.
The pharmacist will also be involved in quality improvement work, audit and practice education. They will have a named pharmacy technician to supervise and will take a lead role in supporting the team. They will assist the technicians and care co-ordinators with medicines reconciliations and medicines monitoring when necessary.
With the support of our executive and HR colleagues, they will contribute towards the appraisal and review process for their team. They will have a GP supervisor with protected time to regularly meet.
Responsibilities
The post holder will be an experienced Pharmacist who acts within their professional boundaries to develop, manage and mentor a team of Pharmacists, Pharmacy Technicians and care co-ordinators to support Adur Health Partnership and Shoreham & Southwick Primary Care Network in all aspects of medicines optimisation.
The post holder will work as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the practice and PCN. This will include running patient clinics that are held face to face and on the telephone for medication reviews, quality improvement projects and medicines safety audits.
The post holder will be supported by existing pharmacists, a GP clinical supervisor and the practice prescribing lead.
The lead pharmacist will ensure that the practice also integrates with community and hospital pharmacy to help improve patient outcomes and ensure better access to healthcare.
The post holder will be required to enrol on the Primary Care Pharmacy Education Pathway (CPPE) if not already completed.

Adur Health Partnership (AHP) is a large, innovative GP partnership based over 4 sites in Shoreham and Southwick which is also one Primary Care Network (PCN) currently serving over 36,000 patients. Our pharmacy team consists of 3 pharmacists, 2 pharmacy technicians and 2 pharmacy care co-ordinators. They are based at the Northbourne site but the pharmacists do clinics across 3 of our sites, which are in close proximity.
