Update: QOF under the 2024/25 Contract

I am writing to all colleagues to provide a reference update for the 2024/25 QOF.

As already circulated, NHS England will income protect a total of 32 Indicators [212 points] for QOF year 2024/25. Income protection will relate to 2023/24 achievement, but payment will be updated to prevalence, list size adjustment, and the increase of the QOF point value to £220.62 reflect 2024.

Of the protected Indicators, 19 are disease registers, which NHS England recommend maintaining via accurately coding patient diagnoses. Doing so also maintains disease prevalence figures. Practices will not be paid additionally for achieving higher outcomes than 2023/24 for any Indicators, however, they will not be penalised for a lower than 2023/24-year end outcome.

The list of protected Indicators is given in section 2.1 of the QOF guidance and are highlighted in grey in the main QOF sections, being, 3 (Clinical Domain) 4 (Public Health) and 5 (Quality Improvement).

In relation to the Vaccination and Immunisation Childhood Immunisation Domain, an automated Personalised Care Adjustment [PCA] has been introduced for VI001, VI002 and VI003 for children registered at the practice too late (in age, or in the financial year) to be vaccinated in accordance with UK schedules. This PCA will be automatically applied on the basis of registration at the practice and removes the child from both the denominator and numerator. These arrangements are explained within section 4.4 of the QOF guidance.

Under Section 4.5 (Cervical Screening) the national screening programme and QOF Indicators (CS005 and CS006) continue to refer to women, and patients registered as male at their GP practice will not receive an invitation for screening via the National Programme, even if they have a cervix.

Practices are encouraged in the guidance to invite such patients for cervical screening directly.
